How involved is the process?

For physicians and allied healthcare professionals, our Credentialing Profile Service has three simple steps:

- Request the Credentialing Services Packet (CSP)
- Complete the CredentialMed enrollment forms
- Submit the Credentialing Services Packet for secure verification and storage.

Upon submission of the complete Credentialing Services Packet, CredentialMed performs primary source verification of the submitted information.

What is primary source verification?

Primary Source Verification involves securing and examining the relevant materials from the originating licensing body, governing institution, board or state/federal regulatory agency. Examples of documentation verified include:

  • Current, valid and unrestricted medical license(s)
  • Board Certifications
  • State-specific certifications
  • Proof of malpractice insurance with effective dates and coverage 
  • US Drug Enforcement Administration actions
  • Federal and State health care programs and Medicare/Medicaid exclusions
  • Clinical privilege actions, sanctions, limitations, or suspensions
  • Professional society membership actions 
  • Current Curriculum Vitae

How much of my time will the credentialing process take?

The credentialing process requires very little time on your part. You must complete and return the documentation and attestations. The remaining verification process is then handled entirely by CredentialMed.

How long does it take CredentialMed to complete my verification?

Our refined process and technological resources enable CredentialMed to process applications quickly. Depending on the extent of verification required, it may take only minutes up to a few days.  Once the credentials are verified, the full report is delivered to the client desirous of your services.

What do you do with my information?

CredentialMed uses your information to verify your credentials on behalf of the CredentialMed client for whom you provide services. We may review and record your information in our database, make inquiries with various credentialing entities, collect and then store verified documents.

Do you share my information?

Your information is stored in our secure database and is considered confidential. CredentialMed will share your information only with the contracted CredentialMed client for whom you provide services. CredentialMed will not share your information with any other party.

How do you store my information?

Your information is electronically stored in a secure, encrypted database, even data-at-rest are encrypted, that can only be accessed by authorized CredentialMed designees. CredentialMed is HIPAA-compliant.

How do I report a change in my credentials?

Many CredentialMed clients require you to provide notification if there is a change in your credentialing status. Please contact CredentialMed at 339-987-9106. You may also notify the CredentialMed client directly.

How do I obtain a copy of the information you have stored for me?

Please submit your request for your information in writing to

Angela.Hynes@CredentialMed.com. CredentialMed will contact you to verify the request. CredentialMed will provide the information no later than 30 days from date the request is verified.

Who has access to the reports?

All reports are confidential and made available only to CredentialMed clients.
